Academic History Workbook
Applicants (including 91社区 students) applying to the MDCM or MDCM PhD program must submit a workbook. Only International Medical Graduates (IMG 2 & 4) and Med-P Qualifying year applicants are not required to submit a workbook.
- Download the Workbook provided and complete it with your academic history, including course history.
- Table 2 of the Workbook should be completed with your entire history of higher-learning (e.g. post-secondary, undergraduate, graduate, etc.).听 This includes advanced secondary programs such as APs from College Board, the International Baccalaureate from IBO, CEGEP programs, and the like.听
- Table 3 need not contain graduate course history.
- Please note some date formats are not compatible with certain operating systems.听 Should you receive an error message, please disregard.
Compatibility: The Workbook was created in Microsoft Excel 2016. The desktop version of Excel is recommended; tables are also compatible with the free version of

Submission
Before submitting your final Workbook, ensure that you rename the file as indicated in Section 6 of the Academic Workbook
You will be able to upload the Workbook in the Applicant Portal after you have submitted your application.听 Be sure to include your 91社区 ID on the Workbook.听 For new applicants, your 91社区 ID will be visible in the Application Portal, top right corner, within 24 hours of your application. If you have a 91社区 ID as a prior student or applicant, continue to use this ID; it will not change.
